What to track

Rate each symptom once a day, from 1 (not at all) to 6 (extreme), plus your period days. Keep going on the good days, too.

Why two cycles

Clinicians use prospective daily symptom ratings (written down each day as you go, not recalled later) for two or more cycles. Two cycles show whether the pattern repeats, which a single month cannot. Read how PMDD is diagnosed for the full explanation.

How to use the sheet

Start any day, use one row per day, and add the date. The row numbers count entries, not cycle days. Mark the first day of your period in the header. Bring both sheets to your clinician.

This is Prova's own chart, not the official DRSP form, and it is not a diagnosis.

Your two-cycle chart

The full chart is below. On a phone, scroll sideways inside each sheet to see all symptoms.

For clean sheets, turn off "Headers and footers" in your print settings.
